Fixed entry for Emerald Green, so that it lists appearances on Kimi ga Ita Monogatari single and .hack//Tasogare no Udewa Densetsu OST.

Grr.... I forgot to save the scan of centre of the Chao Tokyo single.

The edge of the CD cases nearest the edge of the scanner also seem to be missing from the images. I'll try to re-do all of them tomorrow.
