For the 2003 Anime Expo, we can add the musician credits - Konno on violin, Susumu Nishikawa on guitar. Also, the URL has it in 2015-2016 instead of 2003-2004.

Nope. unless there is direct Kajiura involvement. In this case Hikaru just sings herself some Kalafina songs.

@george1234 the number of Kalafina performances where Yuki Kajiura was on stage were very few.

If the set list is available, it's useful to have in the live events section performances of Yuki Kajiura songs by the original or other vocalists who have worked with Yuki Kajiura.
