I have a strong impression that ALL the shapes in question are enlarged in the pic. It isn't that conspicuous with Kyouko and Homura, but Sayaka's PM outfit accentuates her forms. And Mami... Good Lord, let's not get started on Mami. Considering that even her original features are officially a bit overboard (as Ume Aoki herself admitted)...

Could still technically count as "accentuation", but the artist racked that up beyond all reason as well. And it concerns Mami, too, IMHO. I'm not an expert on how women dress, but how does it work to get the shirt's cloth stick so close, er... inbetween? Especially with the given breastsize that pulls it away? It's as if she has a tailor-made one combined with a bra from the start.
Then again, it IS a magical outfit...

Not sure...Looks like some kind of pancake or omelette. Probably eaten at Mami's house in the last episode, I think.

I ignored Mami because it's way too obvious that it's not realistic. In fact, the only realistic one, really, is Kyouko's.

That depends on how close together her boobs are. If her shirt is really, really baggy and she's wearing some SUPER SPANDEX BRA, then it could be that some of it got stuck in her cleave. But it probably wasn't designed to be a baggy shirt.

It's really unlikely, and judging from my friends' outfits when they were buttoned dresses/shirts, it's more realistic to have the buttons or zipper constantly coming undone.

No, definitely not. If she had an outfit designed like a bra, she wouldn't have her boobs sagging so much. Maybe Sayaka stole some perkiness from Mami and everyone else in that picture, except Kyouko, because they all look like they have some sort of phobia of supportive clothing.

Cosplayers these days... they are really not attentive to such important details. Look at this Hidamari Sketch cosplay - the face is NOWHERE NEAR the required width! And cosplaying Madokami without having a sufficiently rectangular face is a true blasphemy.
