I admit, I boggled at the picture at first. 
Well, it IS beautiful, although it's good to have Chiaki's name written on the cover, otherwise good luck recognizing it's her single.

Huh, entry plugs in Evangelion? I don't remember any... but it has really been a while since I watched it.
Somehow it didn't seem like an entry plug to me (maybe because the only entry plugs I've memorised in fiction hail from EXO Squad and Matrix ), rather like it's something placed on her neck. Which it probably is. 
I wonder if it's related to the story.

^when i look at her closely i then realize it's really her.i didn't recognize her at first glance but it's a different chiaki for me.
