I know she left the band at one point, does anyone know why?

Her name is Toyoshima Maya, check here for more info, http://canta-per-me.net/yukis-vocalists/. She sung only in sprinter and during YK vol 2 (where Kalafina did Opening Act).

Next thing we know, she'll reappear and the mysterious 5 vocalists dark song will sum up to quartet!Kalafina + Yuriko. And Sony will put on a trollface.

^ what comes into my mind about the 5 vocalists Yuki going to work with are Kalafina and FJ. Kalafina members and FJ members are 5 person, right? Keiko Kaori Wakana Yuriko and Hikaru. Maybe she is working with FJ for the rumored Sacred Seven OP and with Kalafina for fate/Zero..
