but the title is "The Song" and somehow it says composed by Toshihiko Sahashi

That's from the Symphonic Suite Mobile Suit Gundam Seed album. Orchestral rearrangements of Gundam Seed tracks, I believe. So yeah, that would be the arrangement of Anna ni Issho Datta no ni.

Funnily enough I was at that performance in July 2011 (and spoke to Hiroaki Yura very briefly afterwards).
